printing our Prescription Order form.

Just click the button below to open it.

You can then print it straightaway or copy it to your hard drive.

When you have printed it off just complete your details and send it to
your patients GP.

They will then write a prescription that goes to your patients Pharmacy.

They will then order your patients drinks direct from us. Or via their
Wholesalers Specials Department.
