	Prescription Order Form Print off, complete and send to your resident's GP to obtain more Slõ Drinks® on prescription.
	Data sheet for GP Contains all the essential information your GP may ask you for.
	Drinks pictures The flavours of Slõ Drinks® have been converted to pictures for your aphasic residents.
	Signs and Symptoms of Dysphagia The key signs and symptoms of dysphagia you can use as a reference to monitor your residents.
	Preparation instructions Printed instructions and a video demonstration.
	Kitchen poster Stick it in your kitchen on a cupboard to remind your team you are using Slõ Drinks®.
	Residents on Slõ Drinks® form Write on this form which residents must have Slõ Drinks® and stick it in their kitchen on a cupboard door as a reminder.
	Stage 1: White cup (syrup) stickers Use to remind everyone those in your care need a white cup. Print onto standard Avery Label L7161 and stick in a highly visible place.
	Stage 2: Beige cup (custard) stickers Use to remind everyone those in your care need a beige cup. Print onto standard Avery Label L7161 and stick in a highly visible place.
	Stage 3: Brown cup (pudding) stickers Use to remind everyone those in your care need a brown cup. Print onto standard Avery Label L7161 and stick in a highly visible place.
